Ending Homelessness Team

The Ending Homelessness Team is part of the Homelessness Services Division with the Sonoma County Department of Health Services.  The team serves as the administrative entity for the local Continuum of Care (Sonoma County Homeless Coalition), the designated planning body by the Department of Housing and Urban Development (HUD) for addressing homelessness in Sonoma County.  This also includes operating the Homeless Management Information System (HMIS), Coordinated Entry System, and planning and policy for homeless services.  Other core functions include overseeing contracts for core services in housing, shelter, and outreach as well as evaluating and monitoring local projects funded by state, federal, and local funding streams.  The team monitors system performance for housing placement and retention, returns to homelessness, length of time homelessness, and other core measures.
