Homeless Encampment Assistance & Resource Team (HEART)

Homeless Encampment Assistance & Resource Team (HEART) provides services to Sonoma County’s homeless population and consists of three cohorts: HEART, HOMEKEY, and HOPE. The HEART department is one cohort within the Interdepartmental Multi-Disciplinary Team (IMDT) where integrated care management services are provided for individuals with higher needs.

The HEART and HOPE cohorts provides outreach services to individuals living in encampments in the unincorporated parts of the county and collaborates with local government agencies and Community Based Organization (CBO’s) in the jurisdictions of Sebastopol, Cotati, Rohnert Park, Sonoma, Healdsburg, Cloverdale, Windsor, Santa Rosa and Petaluma to support the outreach needs of these communities.

The HOMEKEY cohort provides service navigation to various Project Homekey shelter and housing sites throughout Sonoma County and assists individuals with connecting to services that are crucial to their well-being and housing stability. In addition, the HOMEKEY team provides case management services to individuals residing at the County’s Interim Shelter Sites.
